Information for New Clients

Please print and complete the first five (5) forms below prior to the initial visit. We will need a copy of your picture ID and health insurance card, if you are using insurance during the check-in process.

 

The forms below are for our fully licensed professionals. Should you need forms for our LPC/A counselors, we can email them to you. It is asked that all new clients arrive 15 minutes early to complete paperwork. Thank you in advance for your cooperation!

Please list your name, phone number, and the best time to contact you. Let us know if you have health insurance and/or Employee Assistance Program (EAP) services with your job, or if you will pay out-of-pocket for services. If you use your health insurance, please call your insurance company before the first visit, and ask about your outpatient mental health benefits, co-payments, co-insurances, and/or deductibles. BAKERS Counseling Services is an in-network provider for the health insurances listed.
